Who we are
Apex Rehabilitation is APEX REHABILITATION AND REFERRALS LIMITED (company 17544604), the veterinary physiotherapy and hydrotherapy clinic at Taylor Vets, Brackley. Contact us at referrals@apexvr.vet or on 01280 422029.
What we keep when you book
Your name, email address and mobile number; your dog's name, breed and age and the problem you describe; your vet practice and whether a vet has seen your dog for it; your answers to our health questions; how you heard of us; and the internet address you booked from, to stop misuse of the page.
Why
To arrange and give the video consultation you asked for, and, with your permission, to talk to your vet: a physiotherapist may treat your dog only with the vet's agreement. We do not use these details for marketing.
Who sees them
- Our veterinary physiotherapists, and the Taylor Vets reception team who answer our phone and run our diary.
- Your vet practice, about your dog's care, as you agreed.
- Google, which runs the video call. We do not record calls.
- The service that sends our emails (Amazon Web Services).
Where and for how long
Your details are kept on our clinic system, on a server in London. If your dog becomes our patient, your details are kept with your dog's clinical record. We keep that for 7 years after their last session.
Your rights
You can ask to see the details we hold, to correct them, or for them to be deleted, and you can object to how we use them: contact us at referrals@apexvr.vet. If you are unhappy with how we have used your details, you can complain to the Information Commissioner's Office, ico.org.uk.
